Speech and Language Pathologist Opportunity, (Fee For Service), with one of the industries’ fastest growing home care companies!
Visiting Nurse Services Westchester, established in 1901, is looking for a Speech and Language Pathologist to serve Dutchess County.

The Speech and Language Pathologist provides comprehensive evaluation, diagnosis and treatment of patients who have communication, speech language, hearing or swallowing disorders

Responsibilities:
  1. Administers speech and language evaluations, as well as some basic informal hearing assessments.
  2. Develops a speech therapy plan of treatment with specific goals in collaboration with the patients, family, physician and health team members.
  3. Provides direct service to treat disabilities through the use of accepted speech, language and swallowing treatment modalities.
  4. Documents all clinical evaluations, treatments and patient’s response to therapeutic interventions.
  5. Continually re-evaluates the patient’s condition and modifies the plan of treatment as appropriate.
  6. Participates in quality improvement activities as appropriate.
  7. Provides education and consultation to Agency staff as needed.
  8. Promotes and facilitates optimal functioning and independence in a therapeutic treatment plan.
  9. Identifies significant clinical information and reports to the physician, Clinical Manager, primary nurse and other disciplines in coordinating services.
  10. Participates in patient care conferences for coordination purposes and documents outcomes.
  11. Instructs and supervises those involved with patient’s care in the treatment modalities appropriate to be carried out independent of the therapist’s visits.
  12. Recommends appropriate interagency and intra-agency referrals.
  13. Provides case management activities for patients/families when appropriate under the direction of the Clinical Manager.
  14. Systematically plans for discharge and continually evaluates the patient’s progress toward this goal.
  15. Instructs and supervises the patient/family and home health aide (and others) in the home exercise program and other aspects of patient care as appropriate and documents their response to instruction.
  16. Provides written home exercise program to patient (and others) as needed and also provides a copy for the patient’s record.
  17. Provides speech therapy only evaluations as directed.
Qualifications:
  1. NYS Professional License and current registration to practice as a Speech and Language Pathologist.
  2. One (1) year of full time experience in the direct provision of speech and language pathology services.
  3. Home care experience preferred.
  4. Automobile in good working order and auto insurance in the amount specified by VNSW.
  5. Valid driver’s license.
  6. Computer Knowledge preferred.
  7. Knowledge of HCHB Software preferred.
  8. Must maintain CEU requirements.
  9. Professional Liability Insurance in the amount specified by VNSW.
Fee For Service Schedule:

Start of Care $150
Evaluation: $90
Regular Visits: $75
