You will work alongside fellow professionals to help our clients live their best possible life!

You will participate in the development of patient physical therapy plans of care; make visits and communicate with the physician before start of care; document the care and submits paperwork in an accurate and timely manner to update and maintain the patient medical records; provide education to patients and their families; participate in regulatory scheduled interdisciplinary team meetings to coordinate the care of the patient, and agency and family;  exchange information and problem solve; and receive staff support and education. 

Requirements include current license as a Physical Therapist (PT) in the state of Florida, current CPR certification, home health experience, Level 2 background screening and recent negative TB/CXR.  Knowledge of principles, concepts and methodology of physical therapy, excellent written and verbal communication skills, demonstrated problem solving and decision-making abilities, and ability to work well with an interdisciplinary team and to develop and maintain rapport with the patient and family a must.  KINNSER/Well Sky experience and at least one year or more work experience in physical therapy in a multidisciplinary setting is preferred.
