Residents, members and even those in the general public, receive compassionate care by an experienced team on a daily basis.

Moorings Park Community Health has been providing Home Health Services to the residents of Collier and Lee counties for over the past three decades. Our team of healthcare professionals and para- professionals are committed to providing Simply the Best services. Home Health service goals are to assist patients in improving function and live with greater independence, promoting an optimal level of well-being. The multi-disciplinary team approach of nurses and therapists focus on assisting the patients in their home so to avoid hospitalization or admission to long term care institutions. Our Home Care team of Private Duty professionals and paraprofessionals focus on maintaining a client’s ability to stay in their home comfortably and safely and in some cases offer respite care to other caregivers. The goal is to ensure the clients receive assistance with their day-to-day activities including personal care needs up to 24 hours per day.
